The Step-On product family is a range of innovative ankle-foot orthoses (AFOs) for the treatment of foot drop. The unique feature of these AFOs is that they are active dynamic, offering mechanical assistance by a spring mechanism to assist dorsiflexion, which can be finely adjusted and quantified. This differentiates them from passive dynamic AFOs which rely on the stiffness of the material to assist / resist movement and with that stiffness being non-adjustable, or from static AFOs which allow no movement or assistance. In this way, Step-On AFOs enable essential kinematics in the sagittal plane during the gait cycle, while providing the exact level of control required by each patient, at every stage of their rehabilitation program.

Foot drop caused by paresis or paralysis due to:

Contraindications

  • Skin lesions / infections
  • Lymphedema
  • History of deep venous thrombosis of the lower limb
  • Ankle-foot contractures
  • Excessive or gross knee hyperextension, as the AFO will struggle to prevent this

Step-On AFOs feature unilateral or bilateral spring hinges. These allow a range of motion (ROM) between 22° dorsiflexion and 11° plantarflexion. This range allows for improved initial heel contact and foot clearance during the swing phase, better step clearance when climbing stairs, and the ability to walk up or down hills, or with a small heel, without affecting tibia alignment. It also allows the knee to exceed 90° in a seated position, which promotes patient independence in sitting and standing.
The adjustable dorsiflexion assist allows for precise adjustment of the AFO based on the patient’s ability to activate their dorsiflexor muscles (to avoid masking this activity), plantarflexor muscle tone, body weight, usual walking speed, etc. It also offers the unique ability to modify the assistance based on the patients’ needs, as their walking ability fluctuates from day to day and even from morning to night. The dorsiflexion assist is easily adjusted using an Allen key that increases or decreases the spring tension inside the hinge.
The Step-On AFOs feature a thin hinge and footplate and come with ¾ length footplates, making them easy to fit into a variety of everyday and city shoes. They can also be combined with foot orthotics (insoles). The shin shell is covered with a lining that can be removed, hand washed and air dried. All hook and loop straps are long enough and can be cut to the appropriate length.

The insoles are not full insoles, but 3/4 insoles. This means that they do not cover the entire foot of a patient, but 3/4 of it. Anyway, if This information is useful to you, here are the measurements of the length of the footbed:

Sizes according to shoe size:
Small : < 39

To determine the size for your personal case:

1- Place your foot flat on a sheet of A4 paper, make a pencil outline, and measure the total length of the foot. Or, if the insole of your shoe is removable, which would be advisable to accommodate the lifter correctly, measure the total length of the sole and send us this measurement in mm.

2- The exact distance between your internal and external malleoli (width between the malleoli),

3- The circumference of your calf at its widest point.
